We bill insurance as a courtesy to our patients. Patients are fully responsible for the bill.

Note that insurance plans change regularly and patients should call for confirmation.

*All office services must be verified/authorized prior to visit.

**We do accept most other plans and nearly all Worker’s Compensation plans.
